Pregnancy is a beautiful journey — but it also comes with unique health needs. Ensuring access to safe and essential medicines can make all the difference for both you and your baby. In Kenya, the Ministry of Health’s Essential Medicines List (KEML) helps ensure that every expectant mother receives the vital drugs and supplements required for a safe, healthy pregnancy.
1. Routine Supplements: Building a Strong Foundation
During pregnancy, your body's need for certain nutrients increases dramatically. The Ministry of Health, following global standards, recommends specific supplements to prevent complications for both you and your baby.
- Iron and Folic Acid (IFA) Tablets: This is the most crucial combination.
- Folic Acid helps prevent Neural Tube Defects (NTDs), serious birth defects of the baby's brain and spine (like Spina Bifida). It is vital to start this before conception and continue for the first 12 weeks of pregnancy.
- Iron is essential to prevent maternal anaemia, which can cause extreme fatigue and increase the risk of poor birth outcomes. You will typically take iron supplements throughout the entire pregnancy.
- Anti-malarial Medication: In malaria-endemic areas of Kenya, the standard protocol includes Intermittent Preventive Treatment in Pregnancy (IPTp), usually with Sulfadoxine-Pyrimethamine (SP). This is given in scheduled doses during your antenatal visits to prevent malaria, which can be severe for both you and your baby.
2. Life-Saving Medicines During Delivery and Postpartum
The KEML includes critical medicines used by healthcare providers to manage obstetric emergencies, which are a leading cause of maternal mortality. Access to these drugs is a key component of quality maternity care.
- Oxytocin: This is arguably the most important drug used during childbirth. It is administered immediately after delivery to help the uterus contract, which is the primary way to prevent and treat Postpartum Hemorrhage (PPH), or severe bleeding after birth.
- Ergometrine (or Methylergometrine): This medicine is also used to prevent and treat PPH, often in cases where Oxytocin alone isn't sufficient.
- Misoprostol: This is another essential uterotonic medicine used for preventing and treating PPH, as well as managing incomplete abortion or miscarriage. It is particularly useful in settings where Oxytocin cannot be safely stored due to its need for cold-chain storage.
- Tranexamic Acid: This injection is now a critical medicine for reducing maternal mortality in women experiencing severe PPH that is unresponsive to initial treatment.
3. Medicines for Managing Pregnancy Complications
The Essential Medicine List also covers key treatments for complications that can arise during pregnancy:
- Magnesium Sulphate: This is the drug of choice for the prevention and management of eclampsia (seizures) in women with severe pre-eclampsia (pregnancy-induced high blood pressure). It is a life-saving medication for this condition.
- Dexamethasone: This corticosteroid is given to the mother in cases of pre-term labour (before 37 weeks) to help speed up the development of the baby's lungs, significantly reducing the risk of breathing problems after birth.
- Tocolytics (e.g., Salbutamol, Terbutaline): These medications are sometimes used for a short period to relax the uterus and temporarily halt premature contractions, giving time to administer Dexamethasone.
What You Should Do: A Call to Action
- Attend All Antenatal Clinics (ANC): Regular ANC visits are where you receive the necessary supplements (Iron/Folic Acid, SP) and where a health worker monitors your health for potential complications.
- Ask Questions: Don't hesitate to ask your healthcare provider about the medicines and supplements you are being given and their purpose.
- Ensure Facility Readiness: When choosing a facility for delivery, you can check that it is equipped for essential maternal care, including having key medicines like Oxytocin and Magnesium Sulphate available.
By understanding the KEML, you empower yourself with the knowledge to demand and receive the high-quality, evidence-based care outlined in the Kenyan health guidelines. Your health matters, and the government has ensured these critical resources are at the core of your maternity care.
